Google should get your review requests, and Yelp should get a claimed page and a light touch. In 2026, BrightLocal’s Local Consumer Review Survey found 71 percent of consumers use Google for reviews, while only 24 percent wrote a review on Yelp in the past year. Yelp also tells businesses not to ask, and its recommendation software filters reviews it suspects were solicited. Google allows requests as long as nothing is offered in return.
That gap changes how a cosmetic practice should spend its front desk time. RealSelf, Healthgrades, Zocdoc, and Vitals each play a different role, and NewBeauty and Allure sit on the editorial side. This post covers the two platforms that most owners confuse: how each treats a review request, what each shows in Google Search, Maps, and AI answers, what Yelp Ads involve, and where the other directories fit.

Why does Yelp filter reviews that Google publishes?
Yelp runs an automated recommendation system, and reviews it does not recommend are pulled out of your star rating. Yelp’s own trust page calls the software “entirely automated” and says it applies the same rules to every business. Google publishes reviews that pass its policy checks and does not sort them by how the reviewer was invited.
Yelp’s trust page says the software looks for unfairly biased reviews, such as reviews people write about competitors. It also states that reviews written when a consumer is asked “tend to be biased compared to reviews that consumers are motivated to write on their own,” and that it looks for reviews it suspects the owner or an employee asked for.
For a cosmetic practice, this produces a familiar frustration. A patient who loved her experience and posted on Yelp after a friendly text from your coordinator may see that review land in the “not currently recommended” section. It still exists, but it does not count toward your Yelp star average.
Google works the other way. Its Maps user generated content policy lets merchants “solicit or encourage the posting of content that does represent a genuine experience, without offering incentives.” Google’s filtering exists, and reviews can be removed for policy violations, but a polite request is written into the policy as acceptable. That single difference decides most of the workflow below.
What are the rules for asking patients for reviews on each platform?
On Google, you may ask, but you may not pay, discount, or reward. On Yelp, the safest reading is do not ask at all. Both platforms sit under a federal layer: the FTC’s rule on fake reviews and review suppression, 16 CFR Part 465, applies to what you do with reviews on either one.
Google’s policy names four boundaries. No incentives such as payment, discounts, or free goods and services. No requiring a review while the person is on your premises. No asking for a certain number of reviews from staff. No requests that dictate what the review says. A conflict of interest, including current or former employment or a contractual relationship, also disqualifies a review. That means staff and family reviews do not belong on your profile.
Yelp’s position is plainer: it discourages solicitation and looks for solicited reviews. A tablet in the consult room pointing patients at Yelp is the practice Yelp says it screens against. The low risk alternative is to claim the page, keep the details accurate, and let patients who already use Yelp write on their own.
The FTC rule adds two lines that matter to any practice. Section 465.2 bars writing, creating, or selling fake consumer reviews. Section 465.7(b) bars representing that the reviews on your site are “most or all” reviews submitted when lower rated ones are being hidden. Do not curate a testimonial widget of five star reviews and call it your review record.
Which platform shows up in Google Search, Maps, and AI answers?
Google Business Profile reviews show directly in Maps and Search. Google’s help center states that reviews “show next to your Business Profile in Maps and Search.” Yelp reviews show up as a separate listing in web results and on Yelp’s own app. Neither platform has published proof that its reviews cause AI assistants to name a practice.
Here is what is documented. BrightLocal’s 2026 survey found ChatGPT and similar tools reached 45 percent usage for business recommendations, the third most popular source. The same survey put Google at 71 percent for reviews. Google’s AI surfaces draw on the business data Google already holds, which is your profile and its reviews. We have not tested whether Yelp review counts change how often ChatGPT or Perplexity names a cosmetic practice, and no published controlled study says they do.
The practical read: Google is the surface every patient touches, so a thin Google profile is the largest hole. Yelp matters where your metro treats it as the local habit. A 2022 Aesthetic Surgery Journal analysis of 11,078 reviews for top rated plastic surgeons across six US metros found Yelp reviews concentrated in Los Angeles, Google in Houston, and RealSelf in Miami, Philadelphia, New York City, and Chicago. Check your own competitors before assuming. Our post on RealSelf vs Google reviews for surgeons walks through that study and the effort split. How much do Yelp Ads cost compared with Google?
Google Business Profile is free to claim, verify, and maintain. Yelp’s business page is free to claim, and Yelp Ads are paid. Yelp’s advertising overview page does not publish a rate card, minimum budget, or cost per click, so any dollar figure you see elsewhere is a third party estimate, and we do not quote one.
The right way to treat Yelp Ads is as a media buy with a tracked cost per booked consult. Ask the Yelp sales team for the current quote in writing, set a test window of 60 to 90 days, and compare cost per consult against your other paid channels. One thing to know before you spend: ads do not change which reviews Yelp recommends. Paying Yelp does not move a filtered review into your star rating, and Yelp’s trust page says its software applies the same rules to every business.
Only Google Business Profile and a verified credential listing are free and fully in your control. RealSelf, Healthgrades, and Vitals sell upgrades without public pricing, and Zocdoc bills per booking.
Where do RealSelf, Healthgrades, Zocdoc, and Vitals fit?
Each covers a different job. RealSelf is where a consideration stage patient checks named surgeons. Healthgrades and Vitals are credential directories. Zocdoc is a booking marketplace. Yelp and Google are general consumer review platforms, and Google is the only one nearly every patient sees first.
RealSelf carries procedure level reviews and provider Q&A, and it limits participation to providers with verified board credentials. Healthgrades and Vitals matter most when a practice also bills insurance, and for accuracy: a wrong address or phone number there undercuts your Google profile. Zocdoc reviews attach to completed bookings, which suits practices with appointment inventory more than cash pay aesthetic consults. | Platform | Cost to the practice | Review request rule | Best for | Recommendation |
|---|---|---|---|---|
| Google Business Profile | Free to claim and maintain | Requests allowed, no incentives, no gating | Every practice, as the default review destination | Primary. Ask every patient here. |
| Yelp | Free claimed page; Yelp Ads paid, no public rate card | Yelp discourages asking and filters suspected solicited reviews | Practices in Yelp heavy metros | Claim and maintain. Do not run a request campaign. |
| RealSelf | Free verified listing; upgrades and ads paid, not published | Verified providers only | Consideration stage proof for surgical procedures | Secondary. Build a complete profile. |
| Healthgrades | Free claimed profile; enhanced options paid | Directory ratings, requests vary | Practices that also bill insurance | Keep accurate. Low effort. |
| Zocdoc | Per booking fee, no subscription | Reviews tied to completed bookings | Practices with open appointment inventory | Test only if you need to fill the calendar. |
| Vitals | Free claimed profile | Directory ratings | Correcting listings that syndicate elsewhere | Audit twice a year. |
| NewBeauty and Allure | Editorial, pricing not published | Not a review platform | Press and authority building | Pursue as earned press, not volume. |
How should a practice split its time between Yelp and Google?
Put nearly all request effort into Google and give Yelp a maintenance schedule. The buckets below assume one to three surgeons and a coordinator with a few hours a week. Move weight toward Yelp only if your metro data shows your competitors’ reviews concentrated there.
1. Google Business Profile: every request goes here
Every consult and post visit ends with one request pointing to one Google link, with no reward attached. Reply to each review in general terms. BrightLocal’s 2026 survey found 74 percent of consumers only read reviews from the last three months, so recency beats a big lifetime count.
2. Yelp: claim, correct, respond, do not campaign
Claim the page, confirm name, address, phone, hours, and categories, and add real photos. Reply to reviews that appear. Do not place a Yelp link in a follow up text or a consult room sign. If a patient mentions Yelp on their own, thank them and let it stand.
3. Directories: accuracy pass twice a year
Check RealSelf, Healthgrades, Vitals, and Zocdoc for matching name, address, and phone. Mismatches across directories are the cheapest problem to fix and one of the most common.
4. Paid and editorial: decide with a budget, not a habit
If you test Yelp Ads, RealSelf upgrades, or Zocdoc, set a cap and a cost per consult target before you start. NewBeauty and Allure features are earned or bought press, and they build authority rather than review counts.
Does any of this change how AI assistants describe a practice?
Possibly, and nobody has proven the size of the effect. Review volume, recency, and tone are documented inputs to how consumers judge a practice, and BrightLocal’s 2026 data shows AI tools now rank among the top sources of local recommendations. What has not been published is a controlled test tying Yelp or Google review counts to how often an assistant names a specific practice.
Make your profile consistent everywhere an assistant can read it: same name, address, and phone on Google, Yelp, RealSelf, Healthgrades, and Vitals. Keep recent reviews flowing on Google. Publish service pages and an FAQ layer on your own site. If you want a view of how that layer fits a plastic surgery practice, our AEO for plastic surgeons page describes the approach Subscribe PR takes, and we say plainly that a free profile and a steady review habit come first.
Frequently asked questions
Can I ask patients to review my practice on Yelp?
Yelp discourages it. Its trust page says reviews written when a consumer is asked tend to be biased and that its software looks for reviews it suspects an owner or employee requested. Asking will not break a law, but it can send reviews to the not recommended section. Claim your page, keep it accurate, and put request effort into Google.
Why are my Yelp reviews not recommended?
Yelp’s recommendation software is automated and applies the same rules to every business. It screens for biased or suspected solicited reviews, and it does not publish its exact signals. A not recommended review still exists on your page but does not count toward your star rating. Paying for Yelp Ads does not change that outcome.
Is it legal to remove or hide negative reviews from my website?
Not if you present what remains as all your reviews. FTC rule 16 CFR 465.7(b) bars misrepresenting that displayed reviews are most or all reviews submitted when lower rated ones are being hidden. Section 465.2 bars writing or selling fake reviews. Show a fair mix, reply to negatives calmly, and report only real policy violations to the platform.
Do Yelp reviews show up in Google Search?
Yelp pages can appear as their own results in Google Search, but your Google Business Profile reviews appear beside your profile in Maps and Search, per Google’s help center. They are separate review pools. Google star ratings do not include Yelp reviews, and Yelp ratings do not include Google reviews.
Are Yelp Ads worth it for a cosmetic practice?
It depends on your metro and your cost per booked consult. Yelp does not publish a rate card on its advertising overview, so get a written quote and run a 60 to 90 day test with a spending cap. Ads do not affect which reviews Yelp recommends, so treat them as a media buy, not reputation repair.
Which one do AI assistants trust more, Yelp or Google?
No published controlled test answers that. BrightLocal’s 2026 survey shows AI tools at 45 percent usage for business recommendations, and Google at 71 percent for reviews. Google’s own surfaces read your Business Profile directly. Keep both profiles accurate and current, and do not trust anyone quoting an exact citation lift for either.
The short answer, from another angle
Google is where patients find you and where you may ask for reviews. Yelp is a page you keep accurate and mostly leave alone, because it filters what it suspects you asked for. The other directories are verification and booking channels, not volume engines.
If you want to see how your practice reads across Google, Yelp, the directories, and AI assistants today, request an audit and we will map the gaps.
